Support for life stage transitions involves both short-term and long-term help aimed at improving the participant’s skills and choices. Major life changes, like moving out, starting college, or getting a job, are part of everyone’s journey. These changes can be tough, both emotionally and practically, especially for individuals with disabilities. Adapting to new places and situations can be hard, but through this NDIS support category, Life Care Mission provides various services to assist with life stage transitions. Our aim is to make the transition as easy as possible, while keeping the participant comfortable. We work together with the participant to ensure a smooth move, helping them adjust to their new environment and feel at ease. Some of the services we offer include:
- Help with linking and organizing
- Skill growth and training
- Help in building independent self-care and living skills
- Crisis and conflict resolution
- Assistance with everyday planning and budgeting
- Creating helpful connections in the participant's community
- Linking with local services and programs
